WORK CERTIFICATION:

If applying for Social Welfare Work Certification, you will need to have TWO FORMS submitted to the Dept of Employment and Social Protection (DEASP). We will complete the Med1 form and forward it to them online for you.
You must then contact the Department on 01 2481398 or 1890 800024 or at the email address below and request an IB1 form, we also have them available at Blackpool Medical Centre. You must complete this and return it to the Department.

This email address is only to request that an application form be posted out to you so please supply your name and address. Forms can only be posted to addresses in Ireland.

From the end of March you will be able to apply online at MyWelfare.ie for Illness Benefit.
